At my Tinder Box pipe party last month, I saw an older gent's wife with a pretty lady's pipe-- I'll have to ask her where she got it. This couple has money, so I'm sure it won't be a cheap one... it was beautiful, though, and complimented this gal's style.

...well, they didn't typically, though the pipe/tobacco companies really tried to get them to do so... it certainly wasn't commonplace.
 
There have always been smallish pipes, and the makers have often tried to market them as "ladies pipes", but as Kyle correctly points out, it has never really caught on as a popular pastime with the fair sex.

Of course, Dunhill makes their relatively tiny Group 1 pipes:
In fact, I would think that anything up to and including a Group 3 would be sufficiently petite for a lady.

...and there's at least one Italian marque that makes a couple of lines of relatively small-bowled pipes:
...any of which might be considered dainty enough for the distaff side.
